Frequently Requested Library Forms

Select forms available for download & printing. All forms must be submitted in person at the library.

Adult Library Card Application: For Texas residents age 18+.
Youth Library Card Application: For Texas residents under 18 years. Parent or Guardian must be present to create library account.

Launchpad Tablet Borrower Agreement: Required for checking out Launchpad tablets.
Kindle eReader Borrower Agreement: Required for checking out Kindle eReaders.
Non-Conformance Form: For requesting a reduction of fines or fees due to extenuating circumstances.
Reconsideration of Resources Request Form: For requesting materials to be removed from the collection or relocated to a different area of the collection.
Public Information Request Form: Official form to request public information in accordance with the Open Records Act.
Patron Complaint Form: For submitting a formal complaint to the Library Director.
Program Proposal: For requesting to host a library sponsored program.
